CHWs serve as a link between the clinical setting and the community, increasing access to services and health education. The following are some of the many services provided by the CHWs

Culturally competant education on health and safety issues
Home visits to assess healthcare needs and provide referrals
Disease prevention and healthy living programs
Community education on available healthcare programs
Assistance accessing healthcare and social services
Helping individuals and family set health goals and follow treatment plans to achieve those goals
Patient advocacy
Diabetes support
Medication Delivery
Weekly Elder meals in all locations

Cultural Preservation

Circle of Elders

The Ponca Tribe of Nebraska believes that by creating a Circle of Elders, much of the Ponca history that is being lost every day could be preserved for future generations. The Tribe and the Culture Department invite many Tribal Elders to work together to share their knowledge and wisdom and provide input for ways to share it with current and future generations.

Gatherings are set up at each office site. Elders are invited to provide ways they can help educate other Tribal members, including youth.
